Step 1 of 7 - Community Support Fund Request Form
This request form is for Sunnydale and Visitacion Valley organizations or individuals seeking limited in-kind support such as gift cards, food, event materials, or other resources.
We are transitioning to a formal request process to help ensure transparency, compliance with audits, and accurate tracking of contributions.
Please submit this form at least 2 weeks before your event or program. Submission does not guarantee approval.
While we strive to provide support, please note that our funds are limited and may impact our ability to accommodate all requests.
If selected, I agree to utilize the support as outlined and will submit an impact statement or summary detailing how the support was used, along with photos (if appropriate), within two weeks following the event or program.
Example Impact Statement
The support provided by Mercy Housing was invaluable to our community event held on May 15th, 2025. These resources were distributed to 60 residents of Sunnydale, helping to promote financial health and foster community engagement.
The event featured a workshop on economic mobility, providing attendees with tools and knowledge for better financial management. Feedback from participants indicated a significant increase in their understanding of budgeting and access to local resources.
